Scissor or scalpel frenectomy — A direct surgical release using instruments. This approach allows clear visualisation of the tissue during the procedure. Sutures may be required depending on the tissue thickness and location.

Surgical release with reconstruction — Used for thicker or more complex restrictions, particularly in older children and adults. This typically involves suturing and may be performed under general anaesthesia.
